     Our participation with any insurance plan is subject to change without notice. Additionally, insurance companies offer a variety of plans and may change the names and benefits at any point. A patient’s level of coverage depends on the specific benefit outlined in their plan,

    To ensure that we are in-network with your policy, it is the patient’s responsibility to contact insurance company to obtain the information of participating provider and benefit plan.

Payment options:

• Cash or Credit Card: Always accepted.
• CareCredit: Apply online (www.carecredit.com) or in our office.
• No Personal Check accepted.

Please note: Payment is due at the time of service.

As a courtesy to you, we will bill your insurance company and track claims. Please keep us informed of any changes to your insurance plan. You are responsible for the fees charged by our office, no matter what your insurance coverage may be. Most insurance companies should respond to the claim within four to six weeks. Any remaining cost is your responsibility.
